Serving our local community through continuity of care

We are humbled and proud that we consistently rank as the leading home health care agency in Gallatin County in patient satisfaction, according to Medicare's "Home Health Compare" ratings system. We know, among other reasons, it's because First Choice Home Health provides every type of medical service that’s available for health care in the home making it easier and less stressful on the client when it comes to choosing different services. Starting with the basics, we offer caregivers who are trained in CPR and dementia care. Additionally, most of our caregivers are CNAs (certified nursing assistants) providing you with that higher level of professional training and knowledge. We also provide transportation to and from appointments and errands whether you’re a client or not. Learn more about our transportation services.

If your needs become greater requiring a registered nurse (RN), physical therapist (PT), occupational therapist (OT), and/or speech therapist (ST), we work with your doctor to provide this too. We also work with Medicare to deliver the medical equipment you need to be cared for in your home, and offer a social worker should you need that additional coordination assistance.

Then, when your doctor determines that you’re well enough that you no longer qualify for Medicare-paid home health care, we’ll help you transition to our Medicare Part B outpatient therapy services, also performed in the home. No other health care agency in the area offers the continuity of care in the home that we do. So as your medical needs change, you stay with us—same caregivers, nurses, and therapists. Clients prefer this because there are fewer phone calls and more familiarity with your caregiver team coming in and out of their home.
